MPQ4228-Q issue

Hello, I have the following issue with the MPQ4228-Q: I made a custom board of the MPQ4228-Q, I have followed the exact schematic as in the typical application circuit, I double checked all the values of the components (physically), and it seems to be working fine (I have normal VCC1,2 values, I have 5.1V at Vout and by checking with a QC3.0 tester I manage to get a variable voltage at Vout all the way from 3.6V to 9V using a 13V input, in 20mV increments as negotiated). However, the circuit shuts down in an irreversible state when attempting to draw more than ~3mA of current from it (slightly more than the QC tester requires) - the board cannot provide power to any real consumer (for example, if I plug in a phone the charging animation lights up for under a second and then it disappears and the circuit is irreversibly shut down until I shut down the 13V Vin and then power it up again. What could be the fault condition that triggers this behaviour? I will exclude over temp because I fiddled around with the NTC on board and this did not trigger it.

I also tried increasing the 13V Vin voltage to 20V and I could get variable QC3.0 output voltage from 3.6V all the way up to 12V, but I still encountered the same behaviour when plugging in a real load (e.g. a phone)

Update: the shut down current is actually ~1A (@5V), and also, another weird thing which I noticed was that by even touching a multimeter probe by the FB pin (pin 12), the IC shuts down in the same manner, until power cycling.
