MPM3833CGRH-Z output overvoltage

Hello,

I have a board in which 3.3V is converted off of a lab supply, and using the MPM3833CGRH-Z, 1.2V is converted off of the 3.3V. Both of these voltage supplies are connected to a processor in which 3.3V is VDDIO and 1.2V is VDD (processor core supply).

On two PCBs now, the 3.3V and 1.2V have been shorted inside the processor exposing the core supply to 3.3V (way out of spec, killing the processor).

Is there anything with the provided circuit below that would lead the MPM3833CGRH-Z to fall out of regulation, increase its output voltage. and kill my processor, possibly shorting the two inside?

For more information: On one board, the regulator was removed and the short remained. Diode checking the input to output on the MPM3833CGRH-Z, things were in order. On the second board the short inside the processor is no more (processor dead), and the MPM3833CGRH-Z is now working as expected.
