MPM3550 cover fills with liquid

We’ve assembled boards with the MPM3550 which has the square lid with the small hole to indicate the location of pin 1. Once the board was powered and some heat was present, liquid began bubbling out of the pin 1 hole. Although the power supply continued to operate on the protoboards we are concerned that we missed some special assembly note. Once we built a larger number, we have seen a few of the power supplies fail.
I don’t see anything in the data sheet but is there some special process to ensure that the cover doesn’t fill with liquid during the board wash? If it does, are there reliability concerns? Should we be baking the board to eliminate any residual moisture?
