MP2636 has low battery voltage

We have had a number of boards manufactured with an MP2636 Switch Mode Battery Charger.

After powering up the the voltage at the BATT pin is only 1.4V.

The VB pin is has 10k5 up to VCC and 10k5 down to ground.

What should I be looking for to remedy this problem?
